Hall of Fame Nomination Form

Eligibility Criteria

Have achieved excellence in the Band at the University for a period of at least two academic years and have been officially dissociated as a student for a period of at least ten years,

and / or

Have contributed to the advancement of the Band at the University, and are not classified as students or directors and who may or may not, have matriculated at the University,

and / or

Have after graduation achieved outstanding success in music.

Support Documents

Letter(s) of recommendation should address the nominee’s qualifications, including:

Professional accomplishments

Special honors and recognition

Outstanding achievements at local, state, national, or international levels

If available, nominators may include additional supporting documentation (including, but not limited to: newspaper articles, videos / recordings, etc.)
